Thursday, July 9, 2026 | 2:00-3:00 PM PST LDCT Mega Trial - Preventive Imaging Agentic AI vs Usual Care

The LDCT Mega Trial is an ambitious vision for one of the largest prospective clinical studies ever proposed in artificial intelligence, designed to evaluate whether an Agentic AI-powered preventive imaging ecosystem can improve long-term health outcomes compared with conventional medical care.
The proposed randomized trial plans to enroll 100,000 participants across 250 clinical sites, comparing standard guideline-based care with an AI-enabled preventive care pathway that begins with a low-dose CT scan and continues through automated imaging analysis, personalized risk assessment, physician-reviewed care planning, patient engagement, and long-term follow-up.
Unlike traditional AI studies that focus primarily on diagnostic accuracy, the LDCT Mega Trial is designed around hard clinical outcomes, including all-cause mortality, major adverse cardiovascular events (MACE), late-stage lung cancer, COPD-related hospitalization, fragility fractures, and cost-effectiveness. The trial also incorporates comprehensive safety monitoring, physician oversight, health equity evaluation, and regulatory governance to support future clinical adoption at population scale.
The presentation outlines the complete trial design, pilot phase, governance framework, endpoint strategy, infrastructure requirements, and implementation roadmap, providing a blueprint for how Agentic AI could reshape preventive medicine over the coming decade.
