Agatston-2.0: A New Generation of AI-Based Coronary Calcium Analysis

Agatston-2.0 is a next-generation artificial intelligence framework developed to enhance cardiovascular risk assessment in individuals with Agatston coronary artery calcium (CAC) scores below 100—a population that represents nearly 75% of individuals undergoing CAC scanning and accounts for a substantial proportion of future coronary heart disease events.

Rather than relying solely on conventional calcium scoring, Agatston-2.0 introduces advanced AI-derived biomarkers that analyze the distribution, density, and spatial characteristics of coronary calcification. By combining these novel imaging biomarkers with additional AI-CVD cardiometabolic measurements—including cardiac chamber volumes, visceral fat, and epicardial fat—the platform provides a more comprehensive evaluation of cardiovascular risk.

The presented data demonstrate that Agatston-2.0 can identify high-risk individuals who would otherwise be classified as low risk by traditional CAC scoring alone. The AI-based approach also shows strong correlation with coronary plaque burden on coronary CT angiography (CCTA), enabling more accurate risk stratification and supporting earlier preventive intervention.
